The benefits of planned and coordinated Early Supported Discharge / Community Rehabilitation Services for stroke patients are well understood, its wide scale adoption was recommended in the National Stroke Strategy in 2007.
A review by the National Audit Office in February, 2010 concluded that these services were cost effective and that their scope should be extended to 100% (from 50%) of patients.
Fundamentally, these services are about streamlining the stressful transfer of stroke patients from in-patient care to intermediate care and ideally into a home care setting. When these critical transfers are not handled effectively they can leave patients feeling abandoned.
There are a multitude of challenges that need to be addressed in improving the effectivenenss of these transfers and the care paitents receive in a non hospital setting. Not least the number of different teams / services that need to be coordinated to ensure that the patient is receiving optimal care in the most cost effective way. For example, Community Rehabilitation Services (CRS) could include:
- GP’s
- Specialist and district nurses
- Dietetics
- Social Care teams
- Respite care
- Night sitting services
- Orthotics
- Consultants and other secondary care staff
- Plus many others...
In addition a key source of support is likely to the patient’s own personal care network.
These challenges will become exacerbated as the service is scaled to support a wider patient population, requiring that current approaches (team meetings etc) are augmented with more effective technology solutions.

All of our solutions are deployed on a 'Cloud' computing basis at a data centre that is being accredited to sit on the secure GovConnect network. Because of this approach the solution does not require a big technology integration / implementation project at client sites and can be implemented in a few months.
